Senior Case Manager - follow-up and outcomes (St. Louis, MO)

Clearpath Lantern Property Management Group is a St. Louis property manager with leasing, maintenance and resident services. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in St. Louis. • Applications are reviewed on a rolling basis.
