Senior Case Manager - follow-up and outcomes (St. Louis, MO)

Northwind Birch Care Services Partners provides in-home and community support in St. Louis. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people based in St. Louis. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in St. Louis. • We reply to every applicant.
