Senior Case Manager - follow-up and outcomes (Spokane, WA)

Stonegate Rook Community Partners runs community programmes across Spokane. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 129 people based in Spokane. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in Spokane. • Applications are reviewed on a rolling basis.
