Senior Case Manager - follow-up and outcomes (Seattle, WA)

Bluepeak Works is looking for a Senior Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Help with applications and appointments for family communication. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Seattle. • Applications are reviewed on a rolling basis.
