Senior Case Manager - follow-up and outcomes (Seattle, WA)

We are growing our team at Bluepeak Stone Care Services Associates and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 37 people based in Seattle. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for licensing. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Seattle. • We will tell you what to expect at each step of the hiring process.
