Senior Case Manager - follow-up and outcomes (Seattle, WA)

We are growing our team at Foxglove Bridge Care Services and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 31 people based in Seattle. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Seattle. • Applications are reviewed on a rolling basis.
