Senior Case Manager - follow-up and outcomes (Seattle, WA)

Stonegate Fox Community Partners is a nonprofit in Seattle supporting families with food, housing and job help, and we need a Senior Case Manager to lead our follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Seattle, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
