Senior Case Manager - follow-up and outcomes (San Jose, CA)

We are growing our team at Redstone Ridge Learning Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for school calendars. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in San Jose. • We reply to every applicant.
