Senior Case Manager - follow-up and outcomes (San Jose, CA)

Stonegate Haven Learning Group runs classrooms and tutoring across the San Jose area. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 12 people based in San Jose. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for enrolment. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, Monday to Friday. • On site in San Jose. • We reply to every applicant.
