Senior Case Manager - follow-up and outcomes (San Jose, CA)

Lumen Beacon Learning Group runs classrooms and tutoring across the San Jose area, and we need a Senior Case Manager to lead our follow-up and outcomes. We follow the school calendar and we like people who like learning. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for attendance records. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with attendance records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in San Jose. • We will tell you what to expect at each step of the hiring process.
