Highbridge Linden Care Services is looking for a Senior Case Manager. The job centres on follow-up and outcomes for visit scheduling, and it would suit someone who likes steady, practical work. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Salt Lake City, with some flexibility after the first few months. • We reply to every applicant.
