Senior Case Manager - follow-up and outcomes (Salt Lake City, UT)

We are growing our team at Bluepeak Drift Civic Services and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for case files. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Full-time, on a regular day shift. • On site in Salt Lake City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
