Senior Case Manager - follow-up and outcomes (Sacramento, CA)

Clearpath Summit Property Management is a Sacramento property manager with leasing, maintenance and resident services, and we need a Senior Case Manager to lead our follow-up and outcomes. We are flexible about schedules. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for vendor invoices. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with vendor invoices. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Sacramento. • Applications are reviewed on a rolling basis.
