Senior Case Manager - follow-up and outcomes (Sacramento, CA)

We are growing our team at Fernwood Meadow Community Partners and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 626 people based in Sacramento. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for food distribution. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Sacramento. • We reply to every applicant.
