Senior Case Manager - follow-up and outcomes (Rochester, NY)

We are growing our team at Northwind Thistle Property Management Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Maintenance and leasing staff share information every day. What you will do • Help with applications and appointments for resident events. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Rochester. • We will tell you what to expect at each step of the hiring process.
