Senior Case Manager - follow-up and outcomes (Rochester, NY)

Clearpath Vista Community Partners is a Rochester charity working with residents on basic needs and opportunity, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Rochester. • Applications are reviewed on a rolling basis.
