Senior Case Manager - follow-up and outcomes (Rochester, NY)

Cedar Summit Early Learning Partners runs childcare centres in Rochester caring for infants, toddlers and preschoolers. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 28 people based in Rochester. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for licensing records. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with licensing records. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Rochester. • We will tell you what to expect at each step of the hiring process.
