Senior Case Manager - follow-up and outcomes (Richmond, VA)

Fernwood Ivory Health provides primary and specialty care in Richmond, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 21 people based in Richmond. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for referrals. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Richmond, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
