Senior Case Manager - follow-up and outcomes (Reno, NV)

Clearpath Vista Community Partners runs community programmes across Reno. We are hiring a Senior Case Manager to take on follow-up and outcomes. Flexibility and kindness are expected. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for donor records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Reno. • We will tell you what to expect at each step of the hiring process.
