Senior Case Manager - follow-up and outcomes (remote)

Greenridge Meridian Health Group is looking for a Senior Case Manager. The job centres on follow-up and outcomes for electronic records, and it would suit someone who likes steady, practical work. You would join a small organisation of about 71 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records.
