Senior Case Manager - follow-up and outcomes (remote)

Highbridge Linden Care Services Group is a social-care provider serving older adults and people with disabilities around Indianapolis, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for licensing. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Indianapolis. • We reply to every applicant.
