Senior Case Manager - follow-up and outcomes (remote)

Cedar Cobalt Property Management manages apartment communities around Jackson, and we need a Senior Case Manager to lead our follow-up and outcomes. We are flexible about schedules. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for maintenance requests. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with maintenance requests. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Jackson. • Applications are reviewed on a rolling basis.
