Senior Case Manager - follow-up and outcomes (remote)

Nova North Early Learning is looking for a Senior Case Manager. The job centres on follow-up and outcomes for enrolment, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for enrolment. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Fully remote, with quarterly visits in Philadelphia. • We will tell you what to expect at each step of the hiring process.
