Senior Case Manager - follow-up and outcomes (remote)

Summit Forge Care Services provides in-home and community support in Albuquerque. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a large organisation of about 989 people, working mostly from home. What you will do • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with caregiver training. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Albuquerque. • We will tell you what to expect at each step of the hiring process.
