Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Ironwood Thistle Learning Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for attendance records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with attendance records. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Atlanta. • We reply to every applicant.
