Senior Case Manager - follow-up and outcomes (remote)

Summit Cross Civic Services works with county and city offices around Tulsa. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for public records. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with quarterly visits in Tulsa. • We will tell you what to expect at each step of the hiring process.
