Senior Case Manager - follow-up and outcomes (remote)

Greenridge Lakeside Care Services supports families and individuals in Tucson with personal care and coordination. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 120 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Tucson. • Applications are reviewed on a rolling basis.
