Senior Case Manager - follow-up and outcomes (remote)

Lumen Granite Housing provides affordable homes and support services around Columbus. We are hiring a Senior Case Manager to take on follow-up and outcomes. We value patience and follow-through. What you will do • Help with applications and appointments for lease compliance. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Columbus. • We will tell you what to expect at each step of the hiring process.
