Senior Case Manager - follow-up and outcomes (remote)

Redstone West Care Services Group supports families and individuals in Grand Rapids with personal care and coordination. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Grand Rapids. • We will tell you what to expect at each step of the hiring process.
