Senior Case Manager - follow-up and outcomes (remote)

Ironwood Beacon Community Partners is looking for a Senior Case Manager. The job centres on follow-up and outcomes for client intake, and it would suit someone who likes steady, practical work. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Part-time, about 28 hours a week, spread across three days. • Fully remote, with quarterly visits in Rochester. • We reply to every applicant.
