Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Amberfield Rowan Care Services and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 16 hours a week, mornings. • Remote-friendly, with a monthly team day in Seattle. • Applications are reviewed on a rolling basis.
