Senior Case Manager - follow-up and outcomes (remote)

Summit Sable Property Management looks after rental homes across the San Antonio area, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 110 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for unit turnovers. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in San Antonio. • We will tell you what to expect at each step of the hiring process.
