Senior Case Manager - follow-up and outcomes (remote)

Westbrook Iron Property Management looks after rental homes across the San Diego area. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 121 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for resident events. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with resident events. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in San Diego. • Applications are reviewed on a rolling basis.
