Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Lumen Clear Care Services Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Grand Rapids. • Applications are reviewed on a rolling basis.
