Senior Case Manager - follow-up and outcomes (remote)

Stonegate Cedar Civic Services works with county and city offices around Columbus, and we need a Senior Case Manager to lead our follow-up and outcomes. Benefits and stability attract long tenure. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with quarterly visits in Columbus. • We reply to every applicant.
