Senior Case Manager - follow-up and outcomes (remote)

Foxglove Ridge Early Learning is looking for a Senior Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. Children come first and so does looking after the teachers. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Albuquerque. • We reply to every applicant.
