Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Harborlight Thistle Early Learning and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 45 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • Fully remote, with quarterly visits in Memphis. • We reply to every applicant.
