Senior Case Manager - follow-up and outcomes (remote)

Amberfield River Community Partners runs community programmes across Tucson. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would join a small organisation of about 11 people, working mostly from home. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for volunteer shifts. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Tucson. • We will tell you what to expect at each step of the hiring process.
