Senior Case Manager - follow-up and outcomes (remote)

Amberfield Pine Property Management manages apartment communities around Cleveland. We are hiring a Senior Case Manager to take on follow-up and outcomes. We are flexible about schedules. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for maintenance requests. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with maintenance requests. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Cleveland. • Applications are reviewed on a rolling basis.
