Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Foxglove Nest Community Partners and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 115 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Cleveland. • We will tell you what to expect at each step of the hiring process.
