Senior Case Manager - follow-up and outcomes (remote)

Northwind Umber Care Services is looking for a Senior Case Manager. The job centres on follow-up and outcomes for care plans, and it would suit someone who likes steady, practical work. You would join a large organisation of about 552 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Memphis. • Applications are reviewed on a rolling basis.
