Senior Case Manager - follow-up and outcomes (remote)

Silverline Field Community Partners Group is a Portland charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for donor records. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with a monthly team day in Portland. • We reply to every applicant.
