Senior Case Manager - follow-up and outcomes (remote)

Summit Oak Health provides primary and specialty care in Seattle, and we need a Senior Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Seattle. • We will tell you what to expect at each step of the hiring process.
