Senior Case Manager - follow-up and outcomes (remote)

Cedar River Early Learning is an early-learning provider in Rochester, and we need a Senior Case Manager to lead our follow-up and outcomes. Children come first and so does looking after the teachers. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Rochester. • We reply to every applicant.
