Senior Case Manager - follow-up and outcomes (remote)

Cedar Iron Housing Group is a nonprofit housing provider in Minneapolis, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 67 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for repair requests. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with repair requests. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Minneapolis. • We reply to every applicant.
