Senior Case Manager - follow-up and outcomes (remote)

Rivermark Glen Health Partners is looking for a Senior Case Manager. The job centres on follow-up and outcomes for clinic supplies, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for clinic supplies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Austin. • Applications are reviewed on a rolling basis.
