Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Rivermark Timber Learning Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for school calendars. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Des Moines. • We will tell you what to expect at each step of the hiring process.
