Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Nova Willow Learning Group and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 552 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Philadelphia. • We reply to every applicant.
