Senior Case Manager - follow-up and outcomes (remote)

We are growing our team at Nova Timber Community Partners and adding a Senior Case Manager whose main responsibility will be follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Part-time, about 16 hours a week, mornings. • Remote-friendly, with quarterly visits in Philadelphia. • Applications are reviewed on a rolling basis.
