Senior Case Manager - follow-up and outcomes (remote)

Stonegate Oak Care Services provides in-home and community support in Fort Worth, and we need a Senior Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 90 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Fort Worth. • We reply to every applicant.
